Child and Teen Checkups (C&TC) Rewards
UCare wants your children to be healthy. That’s why we send you gift cards through our Child and Teen Checkups (C&TC) Rewards program for taking steps to make sure your young children are getting the right screenings.
Gift cards are available for children who:*
- Are up-to-date on immunizations at age 2 years.
- Have a blood lead test at age 1 year.
- Have a blood lead test at age 2 years.
To find out how to get a gift card, view and print the gift card vouchers by clicking on the links below. If your child is a UCare member who is eligible for a gift card, UCare will mail you a voucher to send in for a gift card.
