Grant History

​UCare Fund

Established in 1998, the UCare Fund is a community-directed initiative of UCare, the fourth largest health plan in Minnesota. We focus our grant-making on programs and initiatives that improve the health of underserved populations in the Twin Cities and throughout Greater Minnesota with innovative services, education, community outreach, and research.
The UCare Fund will accept Community Grant Applications from March 1, 2013, through April 15, 2013, and Research Grant Applications from March 1, 2013, through May 31, 2013.

Organizations seeking research or community grants should consider if their mission and activities match the UCare Fund's mission and guidelines. Special attention will be given to grant requests for projects that support the following areas: 

Healthy Lifestyles

Projects with a goal of addressing wellness issues, especially those that deal with physical activity, healthy nutrition, and lifestyle behavior changes. Especially interested in projects that look for ways to improve preventive health care and disease management in diverse populations.

Disabilities

Projects with a goal of reducing health disparities for people living with physical or developmental disability.

Medical Home Initiatives

Projects that connect community resources and local clinics and hospitals to improve the health of members.

Access to Health Coverage 

Initiatives that focus on providing comprehensive outreach support services, including referrals and education, to eligible Minnesotans about health care choices and helping individuals gain access to appropriate coverage options.
The UCare Fund Council reviews final proposals and makes funding determinations. Members of the UCare Fund Council include representatives of the UCare Board of Directors, including a Consumer Board Representative.
