Mammogram Reward
(Information for UCare members only)
UCare encourages women age 50 and over to get a mammogram every one to two years.* A mammogram is an X-ray of the breast. It may find a lump in your breast before you can feel it or have any other symptoms. The sooner breast cancer is found, the better the chances that treatment will work. There is no co-pay for mammograms for UCare members.
Earn a $25 gift card
UCare rewards members age 50 to 69 with a $25 gift card when they get a mammogram.** They will be mailed a voucher for a gift card. The voucher below can also be downloaded and printed:

Members should take the voucher to their mammogram appointment and have it signed by the technologist. Then, mail the voucher to UCare to get a gift card.
Eligible UCare’s Minnesota Senior Health Options members can call 612-676-6868 or 1-800-280-7202 (toll free) to get a gift card voucher. Hours of service are 8 a.m. to 8 p.m., seven days a week.
Eligible UCare Connect members can call 612-676-3395 or 1-877-903-0061 (toll free) to get a gift card voucher. Hours of service are 7:45 a.m. to 5 p.m., Monday through Friday.
Eligible PMAP, MinnesotaCare, and Minnesota Senior Care Plus members can also call Customer Services to get a gift card voucher at 612-676-3200 or 1-800-203-7225 (toll free). Hours of service are 7:45 a.m. to 5 p.m., Monday through Friday.
