You must read this important information before you enroll in UCare's MSHO:
- UCare Minnesota is a Medicare Advantage organization with a Medicare contract. This contract is renewed annually. The availability of coverage beyond the end of the current contract year is not guaranteed.
- You may contact Medicare at 1-800-MEDICARE (1-800-633-4227), TTY 1-887-486-2048, 24 hours a day, 7 days a week, for more information about Medicare benefits and services.
- The counties in the UCare’s MSHO service area include: Aitkin, Anoka, Benton, Blue Earth, Carlton, Carver, Cass, Chippewa, Chisago, Cottonwood, Crow Wing, Dakota, Dodge, Faribault, Fillmore, Hennepin, Houston, Isanti, Jackson, Kandiyohi, Kittson, Lac Qui Parle, Le Sueur, Lincoln, Lyon, Marshall, Martin, Mille Lacs, Morrison, Mower, Murray, Nicollet, Nobles, Norman, Olmsted, Otter Tail, Pennington, Pine, Polk, Ramsey, Red Lake, Redwood, Rice, Rock, Roseau, St. Louis, Sherburne, Stearns, Swift, Todd, Wabasha, Wadena, Washington, Watonwan, Winona, Wright, and Yellow Medicine.
- Beneficiaries must continue to pay Medicare Part B premiums if not otherwise paid for under Medicaid or by another third party.
- With few exceptions, covered services must be provided by network providers or be authorized by UCare to be covered.

Members must use network providers for care, except for emergency care, out-of-area renal dialysis, urgent care, family planning, testing and treatment of sexually transmitted diseases, testing of AIDS and other HIV-related conditions, and in-area urgent care when the UCare network is temporarily unavailable. If you obtain routine care from out-of-network providers, neither Medicare nor UCare will be responsible for the costs.

This plan is available to individuals who are age 65 or over, receive Medicaid (Medical Assistance) from the state, have Medicare Parts A and B, and live in the service area.

Eligible beneficiaries can enroll at any time, effective the first of the following month.
- If you are already enrolled in a Medicare Advantage Part D plan, you must receive your Medicare Prescription Drug benefit through that plan until you become a UCare’s Minnesota Senior Health Options member, at which time you will receive your Medicare Prescription Drug benefit through UCare's Minnesota Senior Health Options.

If you have Medical Assistance, you qualify for and are getting Extra Help to pay for your prescription drug premiums and costs. If you have questions about Extra Help, call:

- The Social Security Office at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY users should call 1-800-325-0778; or

You must use network pharmacies to access your prescription drug benefit, except under non-routine circumstances when you cannot reasonably use network pharmacies. You can use the mail order service to fill prescriptions of what we call "maintenance drugs." These are drugs that you take on a regular basis, for chronic or long term medical conditions. The network includes retail, mail order, home infusion, long term care and Indian Health Service/Tribal/Urban Indian Health Program pharmacies. To obtain a list of our network pharmacies or for more detailed information about mail order or UCare's MSHO prescription drug coverage, please call us at 1-800-707-1711 (TTY: 1-800-688-2534).
