Are You Eligible?

To become a UCare Complete member, you must:

• Have a certified physical disability.
• Be at least age 18 and under age 65.
• Have Medical Assistance with or without Medicare Parts A and B.
• Live in Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, or Washington counties in Minnesota. (The counties are shaded in blue in the map below.)
