Important Information
You must read this information before you enroll in UCare Complete.
- UCare Minnesota is a Medicare Advantage organization with a Medicare contract. This contract is renewed annually. The availability of coverage beyond the end of the current contract year is not guaranteed.
- UCare Complete is a health plan designed by UCare and AXIS Healthcare through a program of the Minnesota Department of Human Services (DHS) called Minnesota Disability Health Options (MnDHO).
- If you are eligible for Medical Assistance with a spenddown, you will need to pay your spenddown to DHS every month in order to remain enrolled in UCare Complete. DHS will give you information on how to do this.
- You may contact Medicare at 1-800-MEDICARE (1-800-633-4227), TTY 1-887-486-2048, 24 hours a day, 7 days a week, for more information about Medicare benefits and services.
- The counties in the UCare Complete service area include: Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington.
- Beneficiaries must continue to pay Medicare Part B premiums if not otherwise paid for under Medicaid or by another third party. Members who have MA-EPD must continue to pay their MA-EPD premiums.
- With few exceptions, covered services must be provided by network providers or be authorized by UCare to be covered.
- Members must use network providers for care, except for emergency care, out-of-area renal dialysis, urgent care, family planning, testing and treatment of sexually transmitted diseases, testing of AIDS and other HIV-related conditions, and in-area urgent care when the UCare network is temporarily unavailable. If you obtain routine care from out-of-network providers, neither Medicare nor UCare will be responsible for the costs.
- You can join UCare Complete if you have a certified disability; you are at least age 18 and under age 65; you are enrolled in the Minnesota Medical Assistance Program with or without Medicare Parts A and B; and you live in the service area. Limitations and restrictions may apply to benefits.
- If you are already enrolled in a Medicare Advantage Part D plan, you must receive your Medicare Prescription Drug benefit through that plan until you become a UCare Complete member, at which time you will receive your Medicare Prescription Drug benefit through UCare Complete.
- You may be able to get extra help to pay for your prescription drug premiums and costs. To see if you qualify for getting extra help, call:
- 1-800-MEDICARE (1-800-633-4227). TTY/TDD users should call 1-877-486-2048, 24 hours a day/7 days a week;
- The Social Security Administration at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY/TDD users should call 1-800-325-0778; or
- Your state Medicaid Office.
If you have Medical Assistance, you are already getting this extra help.
