UCare Connect Care Management

When you join UCare Connect, a team of health care professionals will be dedicated to helping you meet your health and wellness goals.

This team will be led by your Case Coordinator who is a Registered Nurse (RN) or Social Worker (SW) that works in your local community. Your Case Coordinator will:

• Be the main person you contact when you have questions about your needs, plan of care, or other services.
• Work with you to create a plan of care. You’ll use the plan of care to meet your health and wellness goals.
• Help you connect with the health care and community services you need to keep you as healthy as possible.

You’ll also have a Health Plan Navigator who is a Registered Nurse (RN) or Social Worker (SW) at UCare. This person will:

• Work closely with your Case Coordinator to best meet your needs.
• Help you find a Primary Care Physician to arrange for preventive care.
• Refer you for enrollment in one of UCare’s disease management programs if you have a chronic disease, such as heart failure, asthma, or diabetes.
• Get answers to your benefit questions.
