UCare Connect (HMO)

UCare Connect provides adults with physical disabilities, developmental disabilities, and mental illness with health care coverage. It’s a unique health plan that combines the benefits and services of Medicare and Medicaid. With UCare Connect, you benefit from:

• Free membership and no monthly dues at the YMCA.
• Dental care that includes up to $300 extra per year for preventive and restorative services not covered by Medical Assistance. 
• Access to UCare Tooth Care (our mobile dental unit), and help scheduling a dentist appointment in 30 days with our See-A-Dentist Guarantee^SM.
• No premium to join the plan.*
• The ability to join or leave the program whenever you like.**
• Dedicated providers in the UCare Connect network who understand and know how to treat your unique needs.
• Transportation to and from services covered by UCare.
