Are You Eligible?

To become a UCare Secure member, you must:

Have Medicare Part A and Part B (by age or disability).
Have one or more of the following chronic conditions†: asthma, cardiovascular disease, chronic obstructive pulmonary disease (COPD), dementia, diabetes, heart failure, or hypertension.
Live in the UCare Secure service area of Anoka, Chisago, Dakota, Hennepin, Isanti, Ramsey, Rice, Scott, Sherburne, St. Louis, Washington, or Wright counties.
Not have end-stage renal disease (kidney failure).

†Physician verification of the chronic condition is required.
