What is UCare?
UCare is an independent, nonprofit health plan providing health coverage and services to more than 175,000 members in Minnesota and western Wisconsin. Visit our About Us page for more information about our organization.
Who can become a UCare member?
UCare offers health insurance plans for individuals who are eligible for Medicare, have a disability, or qualify for coverage through a Minnesota Health Care Program, such as MinnesotaCare or Medical Assistance.

Medicare Advantage Programs
UCare for Seniors – An affordable Medicare plan available in Minnesota and Wisconsin.
Group UCare for Seniors – An affordable Medicare plan offered to the retirees of more than 60 employers.
UCare Secure – A Medicare plan designed to meet the needs of people diagnosed with one or more chronic conditions.
Minnesota Health Care Programs
For individuals and families
MinnesotaCare – A state-subsidized program for people and families without access to affordable health care coverage.
Prepaid Medical Assistance Program (PMAP) – A federally and state-funded program for people and families who meet income and other eligibility requirements. This program provides medical services to Medical Assistance managed care enrollees.
Prepaid General Assistance Medical Care (PGAMC) – A state-funded program for people who meet income and other eligibility requirements. This program provides medical services to General Assistance Medical Care managed care enrollees.
For seniors
Minnesota Senior Health Options (MSHO) – A plan that combines the benefits of Medicare and Medicaid. It is for people ages 65 and older who are eligible for Medical Assistance and are enrolled in Medicare Parts A and B or who have MA only.
Minnesota Senior Care Plus – A federally and state-funded program for people who are age 65 or older who meet income and other eligibility requirements. This program provides medical services to Medical Assistance managed care enrollees.
For adults with disabilities
UCare Connect – A plan designed to meet the unique needs of adults with a certified disability, or developmental disability. It is for people ages 18-64 who are eligible for Medical Assistance (MA) and are enrolled in Medicare Parts A and B or who have MA only.
UCare Complete – A plan designed to meet the special needs of adults with physical disabilities. It is for people ages 18-64 who are eligible for Medical Assistance and are enrolled in Medicare Parts A and B or who have MA only.
How do I become a UCare member?
Medicare-eligible individuals can call us at 612-676-3500 or toll-free at 1-877-523-1518 for more information. Individuals eligible for UCare’s MSHO, UCare Connect, or UCare Complete can call us at 612-676-3554 or toll-free at 1-800-707-1711. How do I compare UCare for Seniors Medicare plans?

How do I find out if my doctor or dentist is in the UCare network?

How do I make changes to my coverage?
UCare for Seniors members should call Member Services at 612-676-3600 or 1-877-523-1515 Toll Free. Group members who have UCare coverage through their employer should call 612-676-6840 or 1-877-447-4385 Toll Free.
How do I view my claim history?
Members can view personal claims information by logging onto our secure members-only site with their user ID and password. Click on Member sign-in, located in the blue navigation bar at the top of the page.
