All HIPAA covered healthcare providers, both individuals and organizations, must obtain an NPI for use in identification and HIPAA standard transactions. Beginning May 23, 2008 only the NPI can be used to identify healthcare providers and individuals.

Submitting NPI Information to UCare
It is important to notify UCare as soon as you have completed the application process and have received your NPI. Please use one of the following methods for submitting your NPI information to UCare, based on your provider type.
Type 1 Individual Providers: Individual health care providers, or Type 1 providers, should use one of the following methods to submit NPI information:

Type 2 Organization Providers: Organization health care providers, or Type 2 providers, should use one of the following methods to submit NPI information or contact UCare directly:
TYPE 1 NPI SUBMISSION METHODS
Individual Practitioner Spreadsheet (Click to download) Individual providers, or organizations submitting Type 1 NPI information for individual providers, may use the UCare Type 1 NPI Submission File (Excel spreadsheet) to submit their Type 1 NPI information. Use this method to submit multiple UCare provider numbers and corresponding NPI information. Once completed, save a copy of your spreadsheet file and email a copy to demographicupdates@ucare.org.
Electronic NPI Upload File (ENUF) Format Individual providers, or organizations submitting Type 1 NPI information for individual providers, may use the ENUF Format file to submit their Type 1 NPI information. The ENUF Format was developed by the Minnesota HIPAA Collaborative; for more information on the Format and its use visit the Collaborative website at www.mnhipaacollab.org.
Administrative Uniformity Committee (AUC) NPI Notification Form (Click to download) Organization providers may use the AUC NPI Notification Form to submit their Type 1 NPI information. Once completed, mail or fax a copy to UCare or email a copy to demographicupdates@ucare.org.
TYPE 2 NPI SUBMISSION METHODS
If your organization is replacing UCare provider numbers with one corresponding NPI, you may use one of the following methods to report your NPI information. If your organization is replacing a single UCare provider number with one or more NPI, or is replacing multiple UCare provider numbers with a single NPI, please contact us directly at demographicupdates@ucare.org.
