UCare - Providers - Provider Maps

In 2008, UCare's expansion of UCare for Seniors, a Medicare Advantage plan, to nine additional counties in north central Minnesota made UCare a truly statewide program.  The UCare for Seniors plan also expanded into 23 western Wisconsin; making UCare in Wisconsin's service area for the first time.

In 2009, the UCare for Seniors plan expanded into three additional western Wisconsin counties:  Sauk, Juneau, and Iowa; making it a total of 26 counties.

In fact, we now serve more than 185,000 members throughout Minnesota and western Wisconsin. We partner with health care providers, counties, community organizations, and other member-directed groups to create and deliver innovative health coverage plans for:

• Medicare-eligible individuals throughout Minnesota, and in western Wisconsin.
• Individuals and families enrolled in Minnesota Health Care Programs, such as MinnesotaCare and Prepaid Medical Assistance Program.
• Adults with disabilities.
• Minnesotans dually eligible for Medical Assistance and Medicare.
