UCare has partnered with Health Services Management, Inc. ("HSM") to manage the utilization management and quality of our member's outpatient and in-home rehabilitative and habilitative therapy services.
Beginning Jan. 1, 2012, prior authorization of therapy services (OT, PT, and ST) is required before the 4th date of service for each thearpy type. Visits 1-3 do not require prior authorization; however, each visit must meet medical necessity criteria.
There are several ways you can submit a prior authorization request to HSM. You can:
