UCare is pleased to announce that effective January 1, 2010 our Pharmacy Benefit Manager (PBM) has changed to Express Scripts, Inc. (ESI) for all UCare beneficiaries. ESI replaced CVS Caremark (formerly RxAmerica). All covered new and refill prescriptions, including mail-order, should be processed through ESI.
UCare and ESI maintains drug formularies for all UCare plans. Below you will find the UCare plans and the links to the Formularies and Prior Authorization and Exception Forms for each of the UCare plans.
