› Car Seat Distribution Tracking Form
Each time you distribute a seat or seats, this form should be completed and submitted to UCare for program tracking.

Review and sign this form together with the UCare member each time you distribute a seat. This form is for your own files and does not need to submitted to UCare.
› CPS Training Reimbursement Form - Electronic form
Complete this form to receive reimbursement for initial Child Passenger Safety (CPS) certification training, as well as fees for recertification. See instructions on the form for details.
*At this time, electronic form submission is only available for the CPS Training Reimbursement Form. For all others, please download and print forms, and fax to 612-884-2222.
