UCare Fund

Established in 1998, the UCare Fund is a community-directed initiative of UCare, the fourth-largest health plan in Minnesota. We focus our grant-making on programs and initiatives that improve the health of underserved populations in the Twin Cities and throughout Greater Minnesota with innovative services, education, community outreach, and research.

In 2011, the UCare Fund will give special attention to grant requests for projects that support the following areas and populations:

• Obesity reduction, including intervention, prevention, and weight education in communities across Minnesota.
• Preventive health care, with emphasis on screening and outreach activities to individuals. Special consideration will be given to proposals that target improvements in:
• Breast cancer screening – women age 40-69
• Colorectal screening – persons age 50-75
• Diabetes

    ♦  Kidney disease monitoring

    ♦  Blood sugar controlled – with an A1C test


    ♦  Cholesterol screening

    ♦  Retinal or dilated eye exam

    ♦  Cholesterol controlled

• Chronic Obstructive Pulmonary Disease – individuals 40 and older

Preference will be given to grants on the above topics that focus initiatives on seniors, individuals with disabilities, or diverse populations.

Organizations seeking research or community grants should consider if their mission and activities match the UCare Fund's mission and guidelines.

The UCare Fund Council reviews final proposals and makes funding determinations. Members of the UCare Fund Council include representatives of the UCare Minnesota Board of Directors, including a Consumer Board Representative.
