Important Enrollment Information

You must read this information before you enroll in UCare for Seniors.

  • UCare Minnesota and UCare Health, Inc. are Medicare Advantage plans with Medicare contracts. Enrollment
    in UCare Minnesota and UCare Health, Inc. depends on contract renewal.
  • You must continue to pay your Medicare Part B premium.

  • Medicare beneficiaries may also enroll in UCare for Seniors through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov.

  • Members must use plan providers for routine care except for emergency care, out-of-area renal dialysis/urgent care, (and in-area urgent care when the UCare for Seniors network is temporarily unavailable), and Point-of-Service benefits. If you obtain care from out-of-network providers neither Medicare or UCare for Seniors will be responsible for the costs.

  • Benefits, formulary, pharmacy network, provider network, premium and/or co-payments/co-insurance may change on January of each year.
