UCare Choices plans are available in 23 counties in Minnesota with premiums and coverage to fit your income and household size. Fairview UCare Choices is available in 10 metro area and northeast counties.
You can choose a doctor or specialist from our broad network of health care providers, including the doctors at Fairview Health Services, featuring more than 400 clinics across the state. Search Find a Doctor to see if your health care provider is in the UCare Choices or Fairview UCare Choices(SM) network.

It's easy to enroll in UCare Choices through MNsure, the Minnesota health insurance marketplace. The open enrollment period is November 1, 2015, through January 31, 2016. If you qualify for a special enrollment period, you can enroll at other times of year.
It's simple. When you need care, or to schedule an annual check-up or screening, make an appointment with your doctor. On the day of your appointment, remember to bring your ID card with you to show you're with UCare. Also bring your card to a network pharmacy to receive coverage for drugs in our formulary (drug list).

Our Customer Services Department can answer your questions. You can review your Summary of Benefits and Coverage and Contract/Evidence of Coverage anytime by logging into the member site.
All plans have an out-of-pocket limit or maximum on what you will pay for in-network, covered services during the coverage period (which is usually a year). Once you've reached the limit, the health plan covers your costs.
