How to Find a Care Coordinator

This information is intended to help members and UCare providers find a MSHO or MSC+, UCare Connect or UCare Connect + Medicare Care Coordinator.

UCare members, their legal representatives or county employees: To locate an assigned care coordinator or delegate, call the Care Coordination Information Line at 612-676-6502 or 1-877-903-0062.

UCare providers: Primary care, specialty care and other professionals involved in the members' care should contact 612-676-6622 or 1-866-242-2497 for assistance with determining the assigned care coordinator or delegate.
