Care management
Support when you need it most
Our care management program provides UCare members with whole-person support for both physical and mental health or substance use concerns. By bringing physical and mental health together, you get better, more integrated care and support. You can also get help with social needs like housing, food access, community resources and more.
What we offer
Our care management program provides:
- Connections to a variety of UCare and community resources
- Support for all members, including special support for pregnant members, newborns and children up to age 18
- Neonatal intensive care unit (NICU) care management
- Care management to ensure you're receiving the best care possible
- Help with transitions of care, specialty services and referrals
Care management is available to all eligible members at no cost.
Care management for children, teens and pregnant members
Care management for children, teens and pregnant members provides support for physical and mental health concerns, help with substance use recovery and connections to community resources.
Care management for adults
Care management for adults provides support for physical and mental health concerns, help with substance use recovery and connections to community resources.
Adult Complex Care Management (CCM)
Care management services for adults with complex health and social needs and their caregivers.
This program assists members and their caregivers in navigating the healthcare system, coordinating care and finding services and resources to improve their health and quality of life.
