Care management for children, teens and pregnant members

Support for you and your child

Care management for children, teens and pregnant members provides a variety of health, wellness and mental health resources so you and your family get the most impactful care possible.

We address the whole person. That means we provide:

  • Support for both physical and mental health and substance use concerns as well as social needs like housing, food access and more
  • Help with care during pregnancy and for newborns and children up to age 18
  • Neonatal intensive care unit (NICU) care management

We are here to support you no matter where you are in your health care journey.

How it works

Our clinically-trained care managers are dedicated to offering a range of support, including help for pregnant members and referrals to specialty services for kids diagnosed with ADD or ADHD, autism, mental health or substance use concerns. They work with your child and the family to complete an assessment and create a care plan tailored to your child’s needs and health goals. They ensure all the pieces of your child's care plan work together by:

  • Acting as a central point of contact
  • Advocating for your child's health
  • Locating community support and resources for basic needs such as food, housing and transportation
  • Coordinating appointments between health providers and more

Who's eligible

Maternal and child care management services are available to UCare members who've been diagnosed with a pregnancy complication, a high-risk pregnancy or need support after a baby is born, such as having a baby in a neonatal intensive care unit (NICU).

Child and adolescent care management services are available to children with a UCare health plan who need additional support for physical or mental health, ADD or ADHD, autism or substance use concerns. We work with family members who may not be UCare members to help coordinate their child's care.

You can choose to opt out of this program at any time.

Getting help

Our team will provide support for you or a loved one at no additional cost to you. If you need help finding resources, scheduling appointments, meeting your health goals or other physical or mental health needs, call the customer service number on the back of your member ID card.

For pregnancy and postpartum questions, call the Maternal and Child Health Program at 612-676-3326 (TTY 711). Leave a voicemail with your name, member ID and phone number and your call will be returned within 24 hours.

If you have general questions or need after-hours help, call the UCare 24/7 Nurse Line at 1-855-260-9708 (TTY 711). 

Call 9-1-1 if you have an urgent or life-threatening concern.
