Medical Assistance (Medicaid) plans for people with disabilities
UCare offers two health plans for people who are age 18 – 64 with certified disabilities.
UCare Connect
UCare Connect (Special Needs BasicCare) is for people who qualify for Medical Assistance.
We are not accepting new members into UCare Connect at this time.
UCare Connect + Medicare
UCare Connect + Medicare (Special Needs BasicCare) (HMO D-SNP) is for people who want to combine their Medical Assistance and Medicare under one plan.
Eligibility
Enroll in UCare Connect + Medicare if you:
- Have a certified disability or are actively receiving services through the Developmental Disabilities (DD) waiver
- Are at least age 18 and under age 65
- Have Medical Assistance and Medicare Part A and Part B
- Live in the UCare Connect + Medicare service area*
