A Medical Assistance (Medicaid) plan for people with disabilities

UCare offers a health plan for people ages 18 – 64 with certified disabilities.

UCare Connect (Special Needs BasicCare) is for people who qualify for Medical Assistance.

Enroll in UCare Connect if you:

Have a certified disability or are actively receiving services through the Developmental Disabilities (DD) waiver
Are at least age 18 and under age 65
Have Medical Assistance
May or may not be enrolled in Medicare and Part D coverage through another health plan
Live in the UCare Connect service area*
