As our bodies evolve, so do our health care needs. UCare offers a wide variety of health and wellness programs for older adults. From diet and exercise classes to home safety and fall prevention resources — this is your one-stop shop for caring for yourself and your loved ones.

Activity tracker plus Personal Emergency Response System device

This easy-to-use activity tracker plus Personal Emergency Response System (PERS) device features a 24/7 emergency call-for-help button. This service is available to members of UCare Connect + Medicare and UCare's Minnesota Senior Health Options (MSHO) plans.

Classes through Juniper

Juniper offers health management and wellness classes for UCare's MSHO plan members that provide education and strategies to prevent falls and manage chronic conditions.

GrandPad

UCare's MSHO, UCare Advocate Choice (HMO I-SNP) and UCare Advocate Plus (HMO I-SNP) members with an anxiety or depression diagnosis can get an electronic tablet designed to help them stay connected with caregivers, family and more.

Lifespark

Lifespark offers guidance and support that empowers you to take control of your health and independence. This service is available to members of UCare Medicare Plans, UCare's MSHO and UCare Advocate Plans.

Mom's Meals

Mom's Meals provides healthy meal deliveries to eligible members who have been discharged from a hospital stay. This service is available to members of UCare Classic, UCare's MSHO and EssentiaCare plans.

One Pass

One Pass® fitness program gives members access to participating fitness locations nationwide, online classes, at-home fitness kits, brain training and more. This service is available to members of UCare Medicare (excluding UCare Advocate plans), UCare Your Choice Plans, UCare Medicare Supplement, EssentiaCare, UCare's MSHO and UCare Connect + Medicare.

Health Club Savings

Join a class, work with weights, swim some laps or try something new. Health Club Savings offers a monthly reimbursement for eligible members who belong to a participating health club. This service is available to members who are 18 years or older and enrolled in UCare Medicare (excluding UCare Advocate plans), UCare Your Choice Plans, EssentiaCare, MinnesotaCare, Prepaid Medical Assistance Program, Individual & Family Plans or Individual & Family Plans with M Health Fairview.

Lutheran Social Services Healthy Transitions

Get support, coaching and resources for the first 30 days after you return home from a hospital or care facility stay. This service is available to members of UCare's MSHO and Connect + Medicare plans. To learn more and see if you're eligible, contact your care coordinator.
