How to enroll in UCare's MSHO

Receive Medicaid (Medical Assistance) through your county

If you are not already enrolled for Medicaid, you need to apply by filling out a paper application and mailing the application or taking it to your county or tribal office.

Sign up for Medicare Parts A and B

You also need to be signed up for Medicare Parts A and B. You must pay your Medicare Part B premium unless it is paid for by the state.

Enroll with UCare

Once you have Medicaid and Medicare, you have three options to enroll. See below for details.






Enroll online

  • You'll need your Medicaid ID number, Medicare number and an email address to get a validation code.
  • If you need more time, you can save your progress and finish the application later. Access your in-progress application through a link in the original validation email.

UCare’s MSHO has been approved until June 30, 2023, by the National Committee for Quality Assurance (NCQA), a non-profit organization dedicated to improving health care quality.

UCare’s MSHO Star Ratings



UCare's MSHO is a health plan that contracts with both Medicare and the Minnesota Medical Assistance (Medicaid) program to provide benefits of both programs to enrollees. Enrollment in UCare's MSHO depends on contract renewal.
