Earn a reward for completing a screening
Minnesota Senior Health Options (MSHO) (HMO D-SNP) and Connect + Medicare (HMO D-SNP) members can earn a $25 reward for completing a Cologuard colon cancer screening. To claim your reward:
- Complete the at-home screening and send the kit back to Exact Sciences
- Exact Sciences will notify UCare that you completed your screening
- Reward dollars will be loaded onto your Healthy Benefits+ Visa® card
- Rewards are subject to change. UCare reserves the right to deny rewards for any reason.

Eligibility
UCare and EssentiaCare members are identified via claims, pharmacy or other records and sent communications on how they may benefit from completing the Cologuard screening kit. If you’re a UCare or EssentiaCare member who is 45 years or older at average risk of colon cancer and you haven't completed a colon cancer screening or colonoscopy in the last year, you may be eligible* to receive a kit. We encourage you to talk with your provider about results and appropriate screenings.
If you’ve had one of the following conditions, talk with your provider BEFORE completing a Cologuard screening kit. You might need a different approach for colon cancer screening.
Talk to your doctor before completing a Cologuard screening if you have:
- Colon polyps
- Colon cancer
- Inflammatory bowel disease such as ulcerative colitis or Crohn’s disease
- A genetic condition in you or a relative that makes you more at risk for colon cancer, such as Lynch syndrome, juvenile polyposis syndrome or Peutz-Jeghers syndrome
- A family member with colon cancer
- An abnormal prior colonoscopy which might require a follow-up colonoscopy
- Any other condition that you’ve been told puts you at greater risk for colon cancer
