UCare has teamed up with Bluestone Physician Services, CareChoice, Genevive, Knute Nelson and Lifespark to offer a health plan for adults living in a nursing home, assisted living or memory care home — with care coordinators to oversee their care every step of the way.

Is UCare Advocate Choice or UCare Advocate Plus right for you?

Plan members:

  • Have Medicare Part A and Part B
  • Live in a participating facility within the 22-county UCare Advocate service area
  • Receive or qualify for nursing home-level care in a skilled nursing, assisted living or memory care home

UCare Advocate plans offer:

  • Complete care focused on your unique needs
  • Primary care where you live — no need to travel for primary care visits
  • A dedicated care coordinator to help watch over your care, overall health and well-being. Care coordinators work closely with your family and other caregivers, health care providers and UCare.
  • Enhanced dental coverage, including a dental allowance and no-cost dental kit
  • No-cost routine eye exam and prescription eyewear allowance
  • Strong and Stable Kit to improve balance and prevent falls
  • Unlimited routine foot care (does not require a specific diagnosis)
  • Over-the-counter (OTC) allowance on items such as cough drops, first aid supplies, pain relief and sinus medication
  • Annual transportation allowance
  • Memory Support Kit for members with dementia
  • UCare Healthy Benefits+ Visa® card pre-loaded with your prescription eyewear, OTC and transportation allowances to spend at your preferred provider or retailer
