Medication Therapy Management (MTM)

One-on-one help from a pharmacist to get the most from your meds

Most UCare members are eligible for a review of all of your medications, including prescriptions, over-the-counter and herbal supplements. Specially trained UCare or in-network pharmacists will review your medications to ensure they are safe, effective, affordable and easy to use — at no additional cost to you.

Medication Therapy Management (MTM) is especially helpful if you have multiple chronic health conditions, take several medications multiple times per day or have high prescription costs or multiple prescribers.

What are your next steps?

We want you to take full advantage of our MTM program to stay healthy and get the most out of your UCare plan. If you are automatically enrolled for a medication review, you will receive a letter from UCare with next steps in addition to phone calls with more information. You can also:

  • Call to schedule an MTM appointment with a UCare pharmacist
  • Call your health care provider to schedule an MTM appointment with an in-network pharmacist

To make an appointment or opt out of the MTM program, or any individual MTM service, call 612-676-6536 (TTY 1-800-688-2534) or toll free at 1‑855‑931‑5272 and select option 2. Hours of operation are 8 am – 4 pm, Monday – Friday.

Two types of medication reviews

These one-on-one visits give you personalized, expert help to ensure your medicines are doing the most for you and fit within your lifestyle. It’s an opportunity to get your questions answered and can be completed over the phone, through interactive video or in person.

Comprehensive medication review

A full medication review annually by an appointment with a pharmacist lasting 20 – 60 minutes. The review ensures that there are no drug interactions, that medications are being taken as intended and that the medications are as convenient and economical as possible to achieve your health goals. Your pharmacist may provide you with a personalized medication list, medication action plan and cover letter after the visit. The pharmacist also will work directly with your doctor to address any medication-related concerns and update your prescriptions, if necessary.

Targeted medication review

Your pharmacist may recommend an ongoing (quarterly) targeted review of medications for specific issues. Your time commitment is minimal, and the pharmacist may mail or fax information to your health care provider if needed.





You may be automatically enrolled

Members of UCare Medicare Plans with a Part D component, UCare’s Minnesota Senior Health Options (MSHO) (HMO D-SNP) and UCare Connect + Medicare (HMO D-SNP) are automatically enrolled if:

  • You take eight or more prescribed or maintenance medications for chronic condition(s)
  • You have at least three chronic health problems, including:
    • UCare: chronic heart failure (CHF), diabetes, dyslipidemia (high cholesterol), chronic obstructive pulmonary disease (COPD), rheumatoid arthritis
    • EssentiaCare: asthma, chronic heart failure, chronic obstructive pulmonary disease, diabetes, dyslipidemia, hypertension, osteoporosis
  • You plan to spend at least $5,330 in 2024 on your Part D medications
  • You are considered at risk for misuse of certain frequently abused prescription drugs determined by us or your previous plan

If you are automatically enrolled for a medication review, you will get a letter from UCare with next steps. You may also receive phone calls with more information about MTM services. Participation is voluntary and you can opt out of the MTM program or any individual MTM service at any time by calling 612-676-6536 and select option 2.

Pharmacists are medication experts who understand how all of the different drugs and supplements work together. Multiple doctors may prescribe medications without knowing all of the prescription drugs or over-the-counter medications that you take. Your pharmacist will review everything to help you feel confident you are using all your medications safely and effectively. They will also answer many types of questions including:

  • Why am I taking these medicines?
  • What time of day should I take my medicines?
  • Are there any drug-interactions that I should know about?
  • Are my medicines working?
  • Are they still indicated for my condition(s)?
  • Am I experiencing side effects?
  • My prescriptions are expensive, are there ways I can save money?

UCare pharmacists provide an average of five recommendations per visit to improve your medication regimen.


UCare Medicare and EssentiaCare members with prescription drug coverage, MSHO members and Connect + Medicare members are eligible for MTM services through a UCare or in-network pharmacist.

UCare Prepaid Medical Assistance Program (PMAP) and MinnesotaCare members who are outpatient and taking prescriptions to treat or prevent one or more chronic medical condition are eligible for MTM services through an in-network pharmacist.

UCare Minnesota Senior Care Plus (MSC+) and Connect (SNBC) members are eligible for MTM services through a UCare or in-network pharmacist.

UCare Individual & Family Plan members are eligible for MTM services through an in-network pharmacist.

No, completing a visit will not affect your prescription coverage. Your medications will continue to be covered based on the UCare drug list (formulary).

Members with a UCare Medicare Plan with a Part D component, MSHO, MSC+, Connect + Medicare and Connect members can schedule an appointment at no additional cost with either a UCare pharmacist or an in-network pharmacist. A UCare pharmacist may contact you to review your medications if you had a recent stay in the hospital. Visits with a UCare pharmacist will be done over the phone. In-network pharmacists may offer services over the phone or in person for you to complete your medication review.

UCare members enrolled in other eligible plans, including PMAP and Individual & Family Plans, can request MTM services from an in-network pharmacists in person or over the phone.

In-network pharmacists may include pharmacists associated with your doctor’s office or community pharmacy. Not all pharmacists or pharmacies are in network for MTM services. Please contact 612-676-6536 and select option 2 if you have questions.

Appointments with a UCare pharmacist can be made through UCare’s Medication Therapy Management Team at 612-676-6536 and select option 2. You can also reach out directly to your health care provider to schedule an MTM appointment with an in-network pharmacist.

For example, UCare Individual & Family Plan with M Health Fairview members should contact a pharmacist through the M Health Fairview network for MTM services, or contact UCare Customer Service at the number on the back of your member ID card to find an in-network pharmacist.


No, the purpose of a comprehensive medication review is to help you get the most out of your medications. Occasionally, a pharmacist may identify other options based on safety, efficacy or cost, but there is no obligation to change any medications. Our pharmacists can reach out to your provider for you or give you more information for your next visit with your primary care provider.

No, our pharmacists work with your providers to help you get the best possible medication regimen. Many doctors recommend and refer their patients for medication reviews.

Eligible members receive annual comprehensive medication reviews and targeted medication reviews at no additional cost.
