Wellness Navigator program

We’ve teamed up with M Health Fairview to offer extra programs for our eligible members and caregivers to help you through times of emotional stress and concern. That’s because your emotional health and well-being are important to your physical health, especially as you age and even more so if you are responsible for the care and well-being of a loved one. You don’t have to see an M Health Fairview primary care provider to qualify for these programs, which are provided at no additional cost.

Wellness Advisor Program

  • For UCare Medicare with M Health Fairview & North Memorial members

Eligible members are paired with a dedicated Wellness Advisor for support, counseling and resources to help manage your emotional wellness and improve your overall health. You can speak with your advisor by phone or video up to six 60-minute sessions per episode at no extra cost.

Wellness Advisors are licensed mental health professionals who help people maintain and strengthen their personal well-being. They give you personalized counseling and support and find resources in your community that can help.

Caregiver Assurance™

  • For UCare Minnesota Senior Health Options (MSHO) (HMO D-SNP) members diagnosed with cognitive impairment, dementia, Parkinson’s, chronic kidney disease, chronic renal failure, acute renal failure, stroke, primary organ cancer or blood cancer

Being a caregiver for a partner or family member can be challenging. That's why eligible members and their designated caregivers get unlimited advice within the plan year, resources and service referrals tailored to your needs, at no extra cost.

Caregiver Advisors are licensed social workers who can make the caregiving journey a little easier. They help with care coordination, service advice and referrals, stress reduction tips and more.
