How to enroll in UCare's MSHO

1		Receive Medical Assistance (Medicaid) through your county or Tribal office If you are not already enrolled in Medical Assistance, you need to apply by filling out a paper application and mailing the application or taking it to your county or Tribal office. Apply for Medical Assistance

2		Sign up for Medicare Part A and Part B You also need to be signed up for Medicare Part A and Part B. You must pay your Medicare Part B premium unless it is paid for by the state. Apply for Medicare Part A and Part B

3		Enroll with UCare Once you have Medical Assistance and Medicare, you have options to enroll. See below for details.

Enroll online

You'll need your Medical Assistance identification (ID) number, Medicare number and an email address to get a validation code.
If you need more time, you can save your progress and finish the application later. Access your in-progress application through a link in the original validation email.

UCare's MSHO has been approved until June 30, 2026, by the National Committee for Quality Assurance (NCQA), a nonprofit organization dedicated to improving health care quality.
